Functional Medicine: Initial Consult ($200)
At Elevate Health and Wellness, we know that achieving optimal health is never a one-size-fits-all journey. That's why we begin with an affordable Functional Medicine Initial Consult.
This comprehensive visit allows us to deeply understand you - your health history, current symptoms and diagnoses, goals, lifestyle factors, and personal needs. It also officially establishes you as a patient in our practice, giving you access to advanced testing, targeted treatments, and a variety of therapeutic procedures designed to support your healing journey.
What Comes Next:
After completing the Functional Medicine Intake, you'll have a clear evaluation plan (if further evaluation is necessary). You will also gain a better understanding of what we can do to help support you.
Most patients choose to move forward with our Functional Discovery Program, which provides structure, accountability, and the most effective results when completed in the recommended timeline.
For those with more limited needs, there is also the option to continue with care on a pay-per-visit basis.
Whether you're looking for a comprehensive program or just occasional support, we'll work with you to ensure your care plan fits your goals and lifestyle.
Functional Discovery Program ($1300)
Patients experiencing multiple symptoms, complex diagnoses, or long-standing health concerns will typically move forward with our Functional Discovery Program. This structured program is designed to uncover root causes and create a clear, personalized plan of care.
The Discovery Program includes four additional provider visits as outlined below as well as our baseline lab evaluation through Quest Diagnostics including 82 biomarkers of functional health.
We frequently also recommend Advanced Functional Lab Testing. Because they are highly individualized, these specialized labs are not included in the program price. However, all recommended testing and associated costs will be clearly outlined in advance, so there are no surprises.
By choosing the Functional Discovery Program, patients save approximately $150 compared to booking these visits individually - while also benefiting from a more cohesive, guided and supportive experience designed to be completed in a 3-4 month time frame.
1 - Biometrics Visit:
This visit includes the following:
-Comprehensive Exam
-DEXA Scan
-Blood Draw (may be in office or at Quest)
-Urinalysis
2 -Report of Findings & Plan of Care
During this visit we will review the findings of your exam, quest labs and DEXA results. Together we will then outline your personalized plan of care
3 - Functional Lab
Review Visit
This is a second report of findings visit where we will review your functional lab test results and will update your plan of care accordingly.
4 - Follow-Up Visit
We will review your progress and make adaptations to your plan of care as needed. Typically booked at the end of the 3rd or 4th month.
Next Steps:
Once your Functional Discovery Process is complete, you'll have a better understanding of what's going on in your body, why it's happening, and what your next best steps are.
From there, we work with you to determine how you'd like to continue care based on your goals, timeline and level of support needed. There is no single "right" path - only the one that fits you best.
At this stage, patients typically choose one of the following options:
- Continue care on an a la carte basis, scheduling visits, testing and treatments as needed.
- Enroll in our membership program where you can "bank" your membership money in exchange for discounts on all a la carte services, treatments and procedures.

A La Carte Options
Some patients prefer a more flexible approach to care. After completing the Functional Medicine Initial Consultation or the Functional Discovery Program, you may choose to continue with treatment on an a la carte bases, selecting individual visits, testing, and therapies as needed.
This option allows you to move at your own pace while still having access to our providers, diagnostic testing, and therapeutic services. Pricing for all visits and services are transparent and discussed in advance, so you can make informed decisions that align with your health goals and budget.
