Functional Medicine Visits

Schedule follow-up visits when clinically appropriate to review your progress, discuss changing concerns, and make thoughtful adjustments to your care plan.

Visits are subject to clinical appropriateness, provider availability, and our reasonable-use guidelines.

Urgent Care Visits

Members may schedule urgent care visits for appropriate concerns addressed within the scope of services offered by Elevate.

Membership does not guarantee same-day availability and does not replace emergency care.

Sports Physicals

Routine sports physicals offered by Elevate are included for the enrolled member.

Savings on Products and Services

Active members receive:

- 20% off eligible supplements

- 20% off eligible nutrient and wellness injections

Clinical Portal Messaging

Members may use the patient portal for appropriate, non-urgent questions related to established care, including:

- Treatment-plan clarification

- Brief medication or supplement questions

- Progress or symptom updates

- Brief follow-up questions

- Medication or supplement refill requests

Our goal is to respond within 48 business hours, Monday through Friday from 9:00 AM to 5:00 PM, excluding holidays and office closures.

Some questions require an appointment. New or complex symptoms, multiple concerns, extensive lab interpretation, or significant treatment changes may not be appropriate for portal messaging.

Never use the patient portal for an emergency or time-sensitive medical concern. Call 911 or seek care at the nearest emergency department if you believe you are experiencing an emergency.

Connected Care Is Not Concierge Care

Connected Care provides ongoing access within the normal structure and business hours of our practice.

It does not include:

- 24/7 or immediate provider access

- After-hours or weekend portal monitoring

- Direct text-message access to providers

- House calls

- Guaranteed same-day appointments

- Emergency medical services
