Functional medicine is rarely a one-visit experience. Your symptoms, medications, supplements, goals, and healthcare needs can change over time.
The Functional Medicine Connected Care Membership gives established Elevate patients a structured way to maintain continuity with their care team, receive clinically appropriate care when needed, and ask appropriate questions between appointments.
Founding Membership $150 per month — must register by September 30, 2026
What's Included
Functional Medicine Visits
Schedule follow-up visits when clinically appropriate to review your progress, discuss changing concerns, and make thoughtful adjustments to your care plan.
Visits are subject to clinical appropriateness, provider availability, and our reasonable-use guidelines.
Urgent Care Visits
Members may schedule urgent care visits for appropriate concerns addressed within the scope of services offered by Elevate.
Membership does not guarantee same-day availability and does not replace emergency care.
Sports Physicals
Routine sports physicals offered by Elevate are included for the enrolled member.

Active members receive:

Clinical Portal Messaging
Members may use the patient portal for appropriate, non-urgent questions related to established care, including:
Treatment-plan clarification
Brief medication or supplement questions
Progress or symptom updates
Brief follow-up questions
Medication or supplement refill requests
Our goal is to respond within 48 business hours, Monday through Friday from 9:00 AM to 5:00 PM, excluding holidays and office closures.
Some questions require an appointment. New or complex symptoms, multiple concerns, extensive lab interpretation, or significant treatment changes may not be appropriate for portal messaging.
Never use the patient portal for an emergency or time-sensitive medical concern. Call 911 or seek care at the nearest emergency department if you believe you are experiencing an emergency.
Connected Care Is Not Concierge Care
Connected Care provides ongoing access within the normal structure and business hours of our practice.
It does not include:
24/7 or immediate provider access
After-hours or weekend portal monitoring
Direct text-message access to providers
House calls
Guaranteed same-day appointments
Emergency medical services
These boundaries allow our team to provide thoughtful, sustainable care to every patient we serve.

Laboratory testing, specialty testing, medications, supplements, injections, imaging, procedures, and other services not specifically identified as included are billed separately.
The Membership Commitment
Connected Care begins with an initial 12-month commitment. After the first year, membership continues month to month until canceled.
This structure allows us to include clinically appropriate visits and ongoing support at a predictable monthly rate while investing in the staff and systems needed to care for our members.
If membership ends before completion of the initial term, included services already received will be reconciled at their regular à la carte prices. Membership payments already made will be fully credited toward that amount.
